Records: date of application; name/designation and occupation of applicant; date and place of birth; residence details; marital status; occupation, residence and circumstances of husband if a widow; name, age, designation and occupation of children and whether receiving any aid; sources of support or income; condition of health; additional circumstances relative to claim.
The applicant also had to sign a declaration and the answers had to be certified by two householders as true.
